feat(pist): Tier 2B — instrumented trace bridge with real transition matrices
- 24/24 theorems produce trace tags - 18/24 have >1x1 transition matrices (was 0 in Tier 2A) - Unique states: avg 3.6, max 8 - Verified proofs: 5.1 avg steps vs Failed: 2.1 avg steps
